    Published in the middle of the COVID-19 pandemic, when there was fierce debate about system changes needed to manage future pandemics, the recommendations prompted many national public health institutes to question the status of their own as well as global surveillance systems. This led the International Association of National Public Health Institutes (IANPHI - a network of 115 institutes in 98 countries - data from Dec 2022) to seek evidence for the bold changes proposed. The Bill and Melinda Gates Foundation (BMGF) launched a ‘Grand Challenge’,2 targeting selected countries to pilot planning of Integrated Disease Surveillance (IDS) systems based on the five principles and, in parallel, funded IANPHI to research the current status of disease surveillance.
